Terms of Service
Please read these terms carefully before using our services.

Agreement to Terms
By accessing or using the services of Haven Integrative Health, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.
1. Medical Disclaimer
The information and services provided by Haven Integrative Health are for informational purposes only and do not constitute medical advice, diagnosis, or treatment. Our telehealth services are not a substitute for in-person medical care or emergency services.
Margaret is a board-certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) and Family Nurse Practitioner (FNP-BC) licensed in the State of Georgia. All medical decisions, including prescribing decisions, are made within the scope of her professional license and clinical judgment.
Emergency Notice: If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not rely on telehealth services for emergency care.
2. Eligibility and Service Area
Our services are available only to individuals who meet the following criteria:
- ✓You are at least 18 years of age
- ✓You are a current resident of the State of Georgia
- ✓You are physically located in Georgia at the time of all telehealth consultations
- ✓You have a valid government-issued photo ID
- ✓You are not currently pregnant or planning pregnancy
- ✓You do not have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
- ✓You do not have severe gastroparesis or a history of pancreatitis
We reserve the right to verify your identity and Georgia residency before providing services. Providing false information about your eligibility may result in immediate termination of services.
3. Telehealth Services
Haven Integrative Health provides telehealth medical weight loss and wellness services through secure video and telephone consultations. By using our services, you acknowledge and agree to the following:
- ✓Telehealth involves the use of electronic communications to enable healthcare providers to diagnose, consult, treat, and educate patients remotely
- ✓The quality of telehealth services may be affected by technical failures, internet connectivity issues, or other factors beyond our control
- ✓Not all medical conditions can be appropriately evaluated or treated via telehealth
- ✓Your provider may determine that an in-person evaluation is necessary and refer you accordingly
- ✓You are responsible for ensuring you have adequate technology and internet connectivity for telehealth visits
You consent to receive healthcare services via telehealth and understand that these services satisfy Georgia state requirements for a valid provider-patient relationship.
4. Prescription Medications
All prescription medications are prescribed only after a thorough medical evaluation and determination that the medication is clinically appropriate for you. Prescribing decisions are made at the sole discretion of the provider based on clinical judgment.
We prescribe compounded medications prepared by a licensed Georgia compounding pharmacy, regulated by the Georgia Board of Pharmacy. These compounded medications are not FDA-approved. See our FDA Disclosure for complete information.
Important: We do not prescribe controlled substances, stimulant-based weight loss medications, or any medication outside the scope of our practice. We do not guarantee specific weight loss results.
5. Payment and Billing
All services are provided on a direct-pay basis. We do not accept insurance, Medicare, or Medicaid. Payment is required before services are rendered or medications are shipped.
Monthly subscriptions are charged on the same date each month. You will receive a 7-day reminder before each renewal. We charge when we ship — not weeks in advance.
All prices are subject to change with 30 days notice. Existing subscribers will be notified of any price changes before their next billing cycle. For full details on refunds and cancellations, see our Refund & Cancellation Policy.
6. Refunds and Cancellations

Not Approved as a Candidate
If Margaret determines during your initial consultation that this program is not clinically appropriate for you, you will receive a full refund of your medication cost within 5 business days. A $50 clinical review fee is retained for the consultation itself.
Canceling Before Your Consultation
Subscription Cancellation
You may cancel your monthly subscription at any time with at least 7 days notice before your next renewal — no fees, no penalties, no questions asked. If you cancel within 7 days of your next renewal, that month will still ship and bill (fulfillment has already begun with the pharmacy). Your subscription will not renew the following month.
Skipping a Month
You may skip any month by texting “SKIP” before your billing date. No fees, no questions asked. Resume whenever you are ready.
No Refunds for Shipped Medication
Once medication has been shipped, we cannot accept returns or provide refunds. Compounded medications are individually prepared and cannot be returned to inventory. Damaged or incorrect shipments are replaced at no charge within 48 hours of reporting.
7. Patient Responsibilities
As a patient of Haven Integrative Health, you agree to:
- ✓Provide accurate and complete medical history, including all current medications, supplements, and allergies
- ✓Inform us of any changes to your health status, including new diagnoses, medications, or pregnancy
- ✓Follow the prescribed treatment plan, including dosing instructions and follow-up appointments
- ✓Store medications properly (refrigerated at 36–46°F) and handle them safely
- ✓Report any adverse effects or concerns promptly via text to 470-274-7571
- ✓Attend scheduled follow-up appointments for dose adjustments and progress monitoring
- ✓Maintain an active Georgia address for medication shipping
Failure to meet these responsibilities may result in discontinuation of services.
8. Intellectual Property
All content on this website, including text, graphics, logos, images, and software, is the property of Haven Integrative Health and is protected by United States and international copyright, trademark, and other intellectual property laws.
You may not reproduce, distribute, modify, create derivative works from, or otherwise exploit any content without our express written permission. You may view and print pages for personal, non-commercial use only.
9. Limitation of Liability
To the maximum extent permitted by law, Haven Integrative Health, its providers, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of our services.
Our total liability for any claim arising from our services shall not exceed the amount you paid for the specific service giving rise to the claim in the 12 months preceding the claim.
This limitation applies regardless of whether the claim is based on warranty, contract, tort, or any other legal theory, and even if we have been advised of the possibility of such damages.
10. Governing Law and Dispute Resolution
These Terms of Service shall be governed by and construed in accordance with the laws of the State of Georgia, without regard to its conflict of law provisions.
Any dispute arising from these terms or your use of our services shall first be attempted to be resolved through good-faith negotiation. If negotiation fails, disputes shall be resolved through binding arbitration in Gwinnett County, Georgia, in accordance with the rules of the American Arbitration Association.
You agree to waive any right to a jury trial or to participate in a class action lawsuit.
11. Changes to These Terms
We may update these Terms of Service from time to time. Changes will be posted on this page with an updated "Last updated" date. Your continued use of our services after any changes constitutes acceptance of the revised terms. We encourage you to review these terms periodically.
